Still waiting my 480.6C. A lot of excuses and I need that document to file my Taxes

I am giving 1-[-] because I can't give a 0!
[-] mom moved to California to live with [-] (I'm her caretaker). [-] mom with her altruistic mindset decided (against our advice) to [-] some of her assets in Puerto [-], "to help [-] economy of [-] Island" (her words).
But now she needs to transfer all her accounts due to her physical and medical needs. Because of this, she requested [-] cancellation of a certificate of deposit she has with Oriental. She called many days and multiple times, and so did I. One main issue is that every time we called to [-] San Patricio Branch [-] call gets immediately transfer to [-] headquarters in Hato [-]. Then, we get "educated" with [-] fact that we need to contact [-] branch and not [-] headquarters. No matter [-] many time we [-] we called [-] branch and THEIR switchboard is [-] one at fault (I'm guessing [-] switchboard is suffering from IT-dyslexia!) we still get [-] reply that WE need to CONTACT or VISIT [-] branch (hello, California is not precisely "walking distance", we are precisely 3,594 [-] away!).
Finally, one representative told us what to [-] and sent us [-] form we needed to fill out in order to cancel [-] CD and get a check for its value.
We did EVERYTHING! And guess what, instead of a check she got a notification that her certificate of deposit was RENEWED!!!
We called again, and again, and again, to DEMAND her check. Surprise, surprise [-] dyslexic switchboard STRIKES BACK!
She needs her money and she can't travel to Puerto [-]!
0 [-] is probably too much!
